The Women’s Soccer Club, powered by the Women’s Charities, Inc. has established a policy regarding the release of names, addresses, phone numbers, and images of club members and members’ families and staff – including referees and coaches. For inclusion in mailings released, brochures, posters, websites, or any other medium, permission needs to be obtained from the individual(s) involved, and in the case of minors, from their parent/guardian. General group pictures of WSC/WCI members, members of their families, staff, etc. without specific identification of individuals are not subject to this policy.
I, ________________________, consent to the use by the Women’s Charities, Inc (WCI)         (Printed Name)and the Women’s Soccer Club (WSC) of any videotape, photograph, slide, audiotape, or any other visual or audio reproduction in which I or any of my family members may appear. I give my permission for WSC/WCI to mail me club & sponsorship information. I understand that these materials are being used for promotion of the WCI/WSC. Such promotional activities extend to member and sponsor recruitment, fund-raising, etc.  I release the staff, volunteers, etc. of the Women’s Charities, Inc/Women’s Soccer Club from any liability connected with the use of my or my family’s picture or voice recording as part of any of the above or similar activities.

AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY

WOMEN’S SOCCER CLUB AND WOMEN’S CHARITIES, INC.
In consideration of being allowed to participate in any way in the Women’s Soccer Club (WSC) and Women’s Charities, Inc (WCI) athletics/sports program, related events and activities, the undersigned acknowledges, appreciates and agrees that:
1. The risk of Injury from the activities involved in the program is significant, including thepotential for permanent paralysis and death and while particular roles, equipment, andpersonal discipline may reduce this risk, the risk of serious injury does exist; and
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown.EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others and assumefull responsibility for my participation; and
3. I willingly agree to comply with the stated and customary terms and conditions forparticipation. If however I observe any unusual significant hazard during my presence orparticipation I will remove myself from participation and bring such to the attention of thenearest official immediately; and
4. I, for myself and on behalf of my heirs, assigns, personal representative and next of kin,HEREBY RELEASE AND HOLD HARMLESS THE "Women’s Soccer Club and the Women’s Charities, Inc. " and their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releases"). WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR loss or damage to person or property. WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE.

WSC Members Get Involved

The Women's Soccer Club (WSC) is dedicated to keeping the WSC run professionally and as streamlined as possible for the enjoyment of all its members while keeping our session fees low. It is challenging because WSC has many expenses. WSC cannot sustain itself on session fees alone which cover fields & referees, technology, supplies, insurance costs and more. As a condition of maintaining membership in WSC, in order to keep fees low and to keep the WSC running smoothly, each member who plays eight (8) games or more in a calendar year is required to get involved a minimum of four (4) hours per calendar year in the WSC or you may choose to buy out for $50 instead. If you complete more than four (4) hours, it does not count towards the next year get involved commitment. You are responsible for finding opportunities to get involved that fit your schedule and fulfill your commitment.
Make sure you are signed up for the WSC e-newsletter that comes out on the first of every month. If you are not receiving it, sign up on the WSC homepage. The e-newsletter will have updates and opportunities that are available for you to get involved to fulfill this commitment. Only those opportunities identified and approved will qualify for credit to fulfill your get involved requirement. The four (4) hour commitment can often times be easily met in one or two fun and easy opportunities.
Note:
You must complete the Get Involved Hours Form which is found on-line and submit it to your TLC (Team Lead Communicator) for approval within 2 weeks of completing your getting involved commitment to receive credit for your getting involved commitment. Once we have an online system your TLC will be able to enter your hours directly on the WSC website.
Upon approval of hours being submitted, you will receive an email that confirms that your hours have been logged in the system. WSC maintains the right to approve or disapprove any activity as meeting a member's commitment.
You can look up your get involved hours on the website at any time by logging into the online system once it is in place. A club-wide memo will be sent out in the October e-newsletter to remind all club members of the four (4) hour commitment upcoming deadline. You are solely responsible for ensuring that you have fulfilled the minimum hour commitment. WSC does not send individual reminders to members who have not fulfilled their commitment.
If the Club records show that your get involved hours commitment has not be met for the calendar year, on January 1st of the following year, you will have a $50 Get Involved Fee added to your account that will be due before the next session. Players with this fee due will not be allowed to play in subsequent sessions until the fee is paid in full. Payment of this fee will be a condition of continued membership. WSC reserves the right to discontinue or terminate a membership for failure to pay this fee.
REVIEW:

*Complete Get Involved Hours Form and submit hours to Team TLC within 2 weeks of meeting your get involved commitment.
*Your team TLC representative must approve hours before they show up in the system.
*If you do not fulfill your get involved commitment, a $50 Get Involved Fee will be added to your account that must be paid in full before you will be allowed to play.
*Questions regarding this policy should be directed via email to your team TLC Representative.
I, Volunteer, understand the above terms of this Agreement, and understand that as a condition of my WSC membership, I am required to get involved for WSC for four (4) hours every calendar year, or I may not be allowed to play the following year and my membership may be terminated.
